The Dental Membership Plan is not an insurance plan and is not intended to act as or replace dental insurance. For an annual renewable fee, our Dental Membership Plan will allow you to obtain many preventative services at no charge and significant discounts on other dental services.

No claim forms. No yearly maximums. No deductions. No activation fees. No age limitations. No pre-authorization requirements. No pre-existing condition exclusions.

A simple annual membership designed to help patients maintain their oral health while making routine dental care more predictable and affordable.

Individual Plan: $549 Per Year

Includes:

2 Dental cleanings per year
2 Dental exams per year
2 Oral cancer screenings
2 Fluoride treatments per year
Routine dental X-rays

10% savings on any additional dental treatment needed

Periodontal Plan: $749 Per Year

For patients diagnosed with periodontal disease or who require
periodontal maintenance.

Includes:

3 Periodontal maintenance cleanings per year
2 Dental exams per year
2 Oral cancer screenings
3 Fluoride treatments per year
Routine X-rays
20% off scaling and root planing

10% savings on any additional treatment needed

Family Plan: +$449 Per Additional
Family Member

Limitations and Exclusions of
the Membership Plan

Final stage denture prosthetic for all on X procedures.
Worker’s compensation or employers liability treatment services.
Third party products which are sold by the practice (toothpaste, toothbrushes, bleaching materials).
Botox or laser esthetic treatments.
No additional discounts or coupons will be applied.

Membership Plan Terms

Membership plan is not dental insurance.
Membership benefits are available only at Delash Dental Design.
Membership fees are due at one time annually. All fees must be paid by cash, check, or credit card.
Memberships are non-transferable.
All payments are non-refundable. Refunds for membership fees will not be issued at any time if the participant does not fully utilize the plan or obtain services within the 12-month period.
Payment in full for any treatments rendered under the membership plan must be made at the time of treatment.
Extended payment plans, including third party financing plans, may not be used in conjunction with this membership plan (ex. Care Credit).
There may be a charge assessed for any missed appointments or appointments cancelled without 24 hour notice, which is not covered under the membership plan.
Membership fees, services and planned discounts are subject to change on an annual basis.
Delash Dental Design reserves the right to cancel or discontinue this plan for any reason at the end of the membership term.
Services not utilized may not be rolled over to the next year or transferred between family members, plan participants, or any other individual.
Plan is auto renewed each calendar year on the date of purchase.
